1. Role as Parent/Guardian
I am the parent or legal guardian of the minor(s) listed below. I authorize their participation in activities and events at PSC. I have reviewed PSC’s Liability Waiver & Assumption of Risk and Code of Conduct and understand the risks involved.
2. Assumption of Risk on Behalf of Minor(s)
I understand that participation involves inherent risks, including the possibility of injury, illness, or death. I voluntarily assume all such risks on behalf of my minor child(ren).
3. Release of Liability
In consideration for my minor child(ren) being allowed to enter and use PSC’s facility, I hereby release, waive, and discharge PSC, its owners, officers, employees, contractors, affiliates, agents, and successors from any and all claims arising from my child’s participation, including claims of ordinary negligence.
4. Indemnification
I agree to indemnify and hold harmless PSC from any claims, damages, or costs arising from my child’s actions or presence at PSC.
5. Supervision Requirement
I understand that minors must be supervised by a responsible adult at all times. I accept responsibility for ensuring my child follows all posted and communicated rules.
6. Security & Media
I consent to PSC’s use of live and recorded video surveillance for safety. I also consent to PSC capturing photos or videos of my minor(s) for safety and limited promotional purposes unless I opt out in writing via info@pittsburghsportscomplex.com.
7. Governing Law
This agreement is governed by the laws of Pennsylvania, with venue in Westmoreland County.
